Here is a case
deep caries lower six mesially
I removed mesial caries…. then i saw bleeding so i decided it is pathological exposure….. bagan to open my acsess from mesial to distal at the same level of exposure and identified my root canals….. then x-ray showed i am at bifurcation!!! and other 2 files at the same distal orifice and last one was along root of 5!!!!
So it became a complicated case:(
My fault: i should’ve started as if am doing normal class 2 operative then according to the previosly known anatomy i find my canals without lowering the floor that may endanger the bifurcation area
to avoid the mistake: when you remove your file and found bleedeing… stop working
to solve the mistake: we have 2 options:
1st: surgical separation at bifurcation area…but never do unless you r professional
2nd: use 2 pieces of cottons one to block mesial canals and other block distal canal so you see bifurcation….apply CaoH on the perforation…. and give yr 2nd visit after 2 weeks
3rd: once u notice the bifurcation…. use MTA ( I see it’s the best)
4th: extraction:) and patient will never comes again loool
